Community Commons participates in the Medicaid-funded Assisted Living Program. This program began July of 2006. Community Commons has participated in this program since October 2006. This program allows seniors more options when deciding on placement.
The Ohio General Assembly passed this budget bill and the Ohio Department of Aging was given the task of creating the program. Additional information can be reviewed at the Ohio Department of Aging website. This program began as a pilot program. The program is resident driven, meaning the resident requests the assessment and the facility of their choice. The Area Agency on Aging completes the assessment when it is determined the potential resident meets eligibility. The facility of the resident's choice completes an assessment after this takes place to determine if a potential resident can be accepted for accommodation.
A person currently living in an assisted living would be eligible to apply after moving to the nursing facility or if he or she is receiving Passport services in the community. Those seniors living in the community must meet Medicaid eligibility and be receiving Passport services or reside in a nursing home.
Once a senior is approved a date for admission is established and services begin. The Area Agency on Aging Case Manager becomes a part of the interdisciplinary team at the facility and assists in creating and implementing a plan of care. The resident enjoys all of the benefits of the assisted living.
